Here are some excision of lesion CPT coding options with their descriptions:
11440 excision benign lesion 0.0 to 0.5 cm 11441 excision benign lesion 0.6 to 1.0 cm 21030 removal of facial bone lesion benign 21040 removal of lesion mandible benign simple 40810 excision of lesion, vestibule 40812 excise/repair mouth lesion 40814 excise/repair vestibule lesion 40816 excision of mouth lesion complex w/muscle 41116 excision of lesion, floor of mouth
Whichever one most closely describes the procedure you performed is the best one to go with!

Hello, we currently have your Dental Writer cloud based software with Apex claim submission software. We were told by Apex that they cannot send any attachments with the claim submission. My question is when billing for the sleep device, don't we need to send soap notes and the HST test with the medical doctors signature? If we do, how do we send this documentation to the insurance company?

Hi Guest, It's always a good idea to send a narrative along with the claim. Since Medical insurance doesn't accept electronic attachments in box 19 on the claim form you can add a note stating "SOAP note mailed/faxed". That way when they process the claim they will know additional documentation has been sent. For sleep appliances the HST is sent during the pre-authorization request.
