As for the ICD-10 (diagnosis) codes - it all depends on why the patient is receiving the services, in other words, what conditions/symptoms are these services treating?
If you'd like to reply with some details about the patient's condition/symptoms, I'm happy to offer you some coding options.

PT is a child who had a trauma to #31. She feel off her bike.
99204-(D0140)Oral evaluation for a trauma on #31. Evaluation is about an hour. 70310-(D0364)cone beam-3D scan of the tooth-This is part of the evaluation 41899-(D0460). pulp test to check the vitality of the tooth-This is part of the evaluation D9230-nitrous oxide. PT is a child who is very anxious about treatment. Will be used during treatment 99152-(D9248)- non IV sedation- PT is a child who is very anxious about treatment. Will be used during treatment.

Ok great. Based on what you described, here are some ICD-10 coding options to consider:
S02.5XXA - Fracture of tooth (traumatic), initial encounter for closed fracture S02.5XXB - Fracture of tooth (traumatic), initial encounter for open fracture S03.2XXA - Dislocation of tooth, initial encounter G89.11 - Acute pain due to trauma V18.0XXA - Pedal cycle driver injured in noncollision transport accident in nontraffic accident, initial encounter

I desperately need to get 11 ADA Codes converted to ICD10 Codes. I had to go through complete oral reconstruction due to Mandibular Tori. My dentist said it killed all my teeth, and I needed to go on Disability from work because of it. I can not get my Dentist to convert the codes for me. I have had the following done: ADA CODE TIMES DONE D7473 DONE 2 TIMES D7210 DONE 21 TIMES D7953 DONE 5 TIMES D5140 DONE 1 TIME D6010 DONE 6 TIMES D6104 DONE 6 TIMES D6052 DONE 1 TIME D5130 DONE 1 TIME D6110 DONE 1 TIME D6091 DONE 4 TIMES D6111 DONE 1 TIME I need the ICD10 codes for those ADA codes so my Disability claim will be approved. Hi TWISTEDWORDS!
To clarify first, ADA codes (dental procedure codes) do not convert to ICD codes (diagnosis codes). ADA codes can be converted to CPT codes (medical procedure codes), and ICD codes are needed on medical claims to indicate WHY the services are being performed (in other words, the conditions/diseases/symptoms that led the necessity of the procedures being performed). Medical insurance claims require both procedure codes as well as diagnosis codes.
Torus mandibularis (mandibular tori) falls under ICD code: M27.0 - Developmental disorders of jaws
Dead teeth fall under ICD code: K04.1 - Necrosis of pulp
Let me know if you need the ADA codes converted to CPT codes. Some medical insurers will accept the ADA codes on the medical claim form when they are medically necessary procedure performed in the dental practice setting, some will not process ADA codes and require them to converted to CPT codes.
