D0393 - virtual treatment simulation using 3D image volume or surface scan

While dental coding is not our forte like medical coding is, it does appear based on the long description of this code it would be appropriate for treatment simulation for orthognathic surgery.

The long description of this code says:
Virtual simulation of treatment including, but not limited to, dental implant placement, prosthetic reconstruction, orthognathic surgery and orthodontic tooth movement.


It does not appear that there is a direct medical crosscode for this procedure (the only treatment simulation codes currently available in the medical coding system are related to radiation therapy).

So, you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use an "unlisted procedure" CPT code and include a narrative report describing the procedure, for example:
41899 - Unlisted procedure, dentoalveolar structures
76499 - Unlisted diagnostic radiographic procedure
21299 - Unlisted craniofacial and maxillofacial procedure
99199 - Unlisted special service, procedure or report

Courtney,

Can we use code D0393 to bill for treatment simulation using 3D image interpretation? How would this code translate to CPT? This is for an orthognathic surgery with Class III malocclusion.

If you received services from a provider that you did not consent to, I would suggest contacting the provider to inquire about it, it certainly could have been billed by mistake and can be corrected.

Dentist billed code D0470 for diagnostic casts but no impressions were taken. They took an intraoral scan without asking and without prefacing that it would be an additional cost. I just figure it was included in the price of the initial exam. Nope. I did not ask for this service and was only in for a cleaning and exam. My insurance doesn’t cover that code. Will I have to pay the dentist that money even though they did not disclose that it wasn’t part of the services that we went over prior to the appointment?

No, not that we are aware of. The impressions and bite registration needed to fabricate a custom made oral appliance for OSA (HCPCS code E0486) are typically considered included the reimbursement for E0486, so would not be billed separately.

Hi- is there a CPT code to bill a digital scan of impressions when billing out a medical DME? For example- E0486 mandibular advancement device to treat Obstructive Sleep Apnea?

Hi Kelly!

We are not aware of a CPT code available for intraoral scanning. Many times the impressions (whether digital or physical) will be considered included in the reimbursement for the primary service as well. If not considered included in the primary service, you could use a miscellaneous code and include a narrative report describing the procedure since there is not a specific one, such as:
41899 - Unlisted procedure, dentoalveolar structures

What dental code would you use to bill an intra oral scan taken to fabricate a custom abutment and temp crown?

Hi Michelle!

We are not aware of a CPT code available for intraoral scanning. However, it is unlikely a medical insurer would cover an intraoral scan for invisalign treatment as that would be considered cosmetic in nature, and not medically necessary.
