Here is the first sentence on the application:
TO ENROLL IN THE MEDICARE
PROGRAM AND BE ELIGIBLE TO SUBMIT CLAIMS AND RECEIVE PAYMENTS, EVERY DMEPOS SUPPLIER
APPLICANT MUST MEET AND MAINTAIN THESE ENROLLMENT STANDARDS.
