I am needing to file Medical Insurance for 4 impacted Wisdom Teeth. I believe I am supposed to use the same code D7240 for the Medical claim. We extracted all four and I'm not sure how to fill out the claim form. I have D7240 with a diagnosis code of 520.6 but is there a modifier needed for anything? Is the modifier where the insurance company knows that all four were extracted? What will the claim form look like for all 4 teeth extracted? One line for each tooth, a modifier? HELP!
