Hello, I have a patient that has been wearing a flex appliance and lost the bottom portion. Do we treat it like a new appliance? We didn't give her the one she has it was done over 5 years ago in another state. She hasn't had a sleep study done in 6+ years. Patient says her insurance paid for it last time.

If patient had an oral appliance created 5yrs ago and her sleep study was done 6yrs ago. It would be best to have an updated sleep study to get an accurate baseline for the patient. Ins. companies will typically cover a new appliance every 5 years. I would suggest doing a pre-auth.
