What CPT procedure code , diagnosis code and modifiers should be used for Botox injections on medical claims?

Hi jhalmaghi! The procedure and diagnosis code to use for Botox injections will depend on the nature of the treatment - i.e. why were the botox injections medically necessary for this patient? J0585 is a code for the Botox, but the rest will depend on the patient's situation.
