Our office has received an appeal denial from a medical ins for 21048. Reason: The billed procedure 21048 was denied because it was performed in an office setting.

I have never seen this code deny before. Any suggestions?

21048 stands for: Excision of benign tumor or cyst of maxilla; requiring intra-oral osteotomy (eg, locally aggressive or destructive lesion[s])

What insurer did you receive this denial from? Is it possible the patient's medical plan only covers procedures performed in a facility setting?
