Do I need to use a modifier with code 40819 for labial frenectomy? Is the diagnosis code 27.41?

There is not a standard modifier we are aware of for 40819 - Excision of frenum, labial or buccal (frenumectomy, frenulectomy, frenectomy)

You may consider modifiers such as 51 (multiple procedures) if there were other surgical procedures performed on the same date of service, or modifier 22 (increased procedural services) if it was an usually difficult case, for example.

As for the diagnosis - 27.41 is no longer valid (it was an ICD-9 PCS diagnosis code, and the current version of diagnosis code is ICD-10).

That code converts over to ICD-10 PCS as:
0CN00ZZ Release Upper Lip, Open Approach
or:
0CN03ZZ Release Upper Lip, Percutaneous Approach


However, if you are billing from an office setting, you may be looking for ICD-10:
Q38.0 - Congenital malformations of lips, not elsewhere classified


For a lingual frenectomy:
Q38.1 – Ankyloglossia
