Do I enter 45 minutes into field 19 or the elapsed time shown on the anesthesia record which is 40 minutes?

Yes absolutely, you want to list 00170 just once on the claim and increase the units on the line item to 3. If BCBS still says they want the amount of time specified, I would suggest entering the total minutes in field 19.

I am using code 00170 as follows:

00170 Deep sedation/gen. anesthesia 1st 15 mins 1 unit
00170 Deep sedation/gen. anesthesia add. 15 mins 1 unit
00170 Deep sedation/gen. anesthesia add. 15 mins 1 unit

Should I be listing this code once with 3 units in field 24G for 45 minutes total anesthesia time? My software lists each 15 minute unit separately. BCBS keeps denying my claims for missing information: anesthesia time required. I'm at a loss and cannot get my Delta of NJ claims paid without a medical EOB.

Great question. Typically this is reported by 1 unit per 15 minute increment in field 24g. Is that something you are already entering?

I have routinely submitted medical claims when necessary for dental reimbursement for many years. Recently my medical claims to BCBS are being denied due to: Anesthesia Time Required. Any advice on where the anesthesia time should appear? It is clearly indicated on the anesthesia record. Should it be on the claim form somewhere?
