D0150 - comprehensive oral evaluation - new or established patient
can be crosscoded to:
99202 - level 2 new patient Evaluation & Management (office visit)
or
99212 - level 2 established patient Evaluation & Management (office visit)


D7140
- Extraction, erupted tooth or exposed root (elevation and/or forceps removal)
D6548 - Retainer - porcelain/ceramic for resin bonded fixed prosthesis
D6245 - pontic - porcelain/ceramic
D0470 - diagnostic casts

The services listed above actually do not have direct crosscode - so you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use the CPT code below and include a narrative report describing the procedure:
41899 - Unlisted procedure, dentoalveolar structures


You will also need ICD-10 (diagnosis) codes to describe the injury to the teeth and how it occurred. Here are some commonly used ICD-10 codes for injury to teeth:

S02.5XXA - Fracture of tooth (traumatic), initial encounter for closed fracture
S02.5XXB - Fracture of tooth (traumatic), initial encounter for open fracture
S03.2XXA - Dislocation of tooth, initial encounter
G89.11 - Acute pain due to trauma

You will also need a code to describe how the accident happened. If you'd like to provide me with some details about the accident/injury, i'm happy to provide you with some coding options.

My husband had an injury to front tooth and it had to be removed and a bridge created.

Since it was due to an accident, I would like to get the medical codes that relate to the dental procedures commpleted

Also went to a facility that has used codes
150 - Comprehensive Oral Eval
470 - Diagnostic Casts
